People can have lots of illnesses that are "unseen" up till a point. I noticed that John Watts family claim he was a healthy individual and using your powers of assumption, going by the first picture he could very well be a steriod using body builder. We all know how much strain is but on the body by body builders.
Also as Member Chr0naut posted, he has tatoos. If the needles used weren't sterilized correctly that could also cause health problems. (back street tatoo artists use very old products called a Little Sister sterlizer)

And again, you make the assumption that his family knows if he has any underlying unseen medical conditions. You seem to assume they are the medical experts here.
John Watts and his family know better than me and you and everyone else. No diseases as far as they are concerned. They don't have to be experts to know about John Watt's general health.
You can only make assumptions and a hypothesis at this stage given that there is no proof or conclusion.
