Most likely, he was pointing the gun at the camera, and the director and cinema photographer were behind the camera.

The actor isn't, or shouldn't be, anywhere near that.

That's the function of the specialist they have, or should have, on set whose primary mission is to make sure those props being used are safe.

Because it's a movie. The gal that died, she runs the camera. He was shooting in the direction right into the camera (which is done all the time in movies).

Generally the director is watching over the shoulder of the camera person so they can see it as it appears on video. I don't know enough on how blanks work but from doing some reading a malfunction of sorts could cause the blank to essentially split (IDK, like I said, just don't know enough about that stuff).

Yeah you can go ahead and trust the system to "look into it" and then when they tell you its all on the up and up believe it .

But I choose to question how he shot 2 people with a prop gun on accident.
