Speculating here.

It's a Western and the two people shot were the director and cinematographer. I'm thinking it was a scene where the actor was pointing a gun at the camera and fired hitting both. It may very well be a shotgun as if this were a single action replica revolver you'd have to cock and then recock the hammer to get two shots off and it should have been obvious prior to that if someone was hit by a live round.

I’ve read in an article that 2 projectiles got discharged at once.. hitting the 2 victims. I have no idea about guns but maybe a dirty barrel or pieces of the (blank) bullet casing?

Prop guns don't just make fun noises, they actually expel a projectile designed to be "harmless". Which is still incredibly stupid.
