If it's real, the only thing it really resembles is a leech but it would be about 4x larger than the largest leech species known and probably 20-30x heavier.
It's not impossible that it's a species of heretofore unknown leech that just happened to find it's way into a toilet in a couple's house but I'd call it improbable.
It moves more like a snake imo. Perhaps it's something like a snake in a nylon?

Looks like an intestinal worm someone released into the toilet during one of their bathroom motions, that just continued to grow and develop from all the food it was continuing to receive while clinging to the pipe system there. When it got too big to live in the pipes, it just crawled back up and out the lou.
