That my friend is easy to do.... You host the image on your own computer as the server, and you swap the picture with matching file names.

Congratulations, the picture that loads in the old post is now a new picture. This does not allow you to edit the text of the original post.

Or, you are reffering to avatars, in which case, the same exact method is applied, just with Twitters servers instead of your own.
