It is irrelevant, the state would become an independent republic and have its own currency. The state would file bankruptcy if they owe any money to the Federal government, or simply pay them off, because their currency would be worthless to the state from now on.

So there will be a small number of radicals who will try to start a rebellion and get beat up, but their medics don't support the new health plan and will refuse to treat their wounds because craziness is a pre-existing condition. Then they'll be in prison, where health care is freely available, and perhaps can cure them of their brainwash and allow them to be sane citizens once again.
