As pointed out, look at Tenerife. There were multiple factors involved in that, but the KLM pilot that ultimately caused it, Veldhuyzen van Zanten, had over 11,000 hours total, and over 1500 in the 747. He was the head of the flight training department for KLM, and responsible for training all 747 pilots for the airline. Pressure caused by the crew scheduling system pushed them to get to their destination before they ran out of duty time, and possibly pushed him to get into the air to get there.

Any pilot can make mistakes, or be pressured into doing something they know they shouldn't, especially flying a VIP. We saw it to a smaller degree with the two C-135s based at Hickam in the 80s. They were taking off one day and had a fire warning light on one of the engines, and returned. They were taking off again a couple hours later, with the general in the right seat, and got the fire warning light again. The pilot flying pulled that throttle back, the light went out, he pushed it forward and it stayed out. The general looked at him, and said "What do you want to do Captain?" When the Captain said they'd keep going, he said "Good choice". That pressure is going to be even worse for someone like a President, who is used to being able to say "Go" and whatever he's in goes.

Two pressure scenarios. Senator Ron Brown had to get in... I knew the Pilot very well... logged many hours with him and he was a good pilot, the other was Kobe, and the pilot had crazy hours.
