Freshwater lakes are formed in any of several ways from tectonic processes to glacial lakes to volcanic activity to sinkholes. The concentrations of dissolved solids(including salts) varies depending on local geography. There are some large inland bodies of water that were never connected to the sea that are salt water formed as described by Phage there are many others that have varying concentrations of dissolved solids considered to be freshwater. Its a lengthy scale of variation as opposed to an either or proposition. A little light reading for reference-
