A lot of times, taking a snippet out of one of his commentaries is an easy gotcha when the full text takes a page or more and sheds a lot of light on things.
I'll bet the caller was an angry woman too, either that or he wasn't making the comment at her directly.
Media Matters puts most of those out, and the usually picked them to worst effect and lot of them weren't exactly what they were portrayed to be.

Gonna paraphrase Bette Davis. "You should never say bad things about the dead, only good… Rush Limbaugh is dead. Good.”
Bo Snerdley
@BoSnerdley
I just saw the footage from CNN's Jim Acosta saying Rush has a "history" of saying disparaging things about African Americans. I have been in the studio with Rush for 30 years. I would like to formally challenge CNN and
@Acosta to provide the list.

Glad he is dead. Wish he would've died sooner.
"When actor Michael J. Fox, suffering from Parkinson’s disease, appeared in a Democratic campaign commercial, Limbaugh mocked his tremors. When a Washington advocate for the homeless killed himself, he cracked jokes. As the AIDS epidemic raged in the 1980s, he made the dying a punchline. He called 12-year-old Chelsea Clinton a dog.
He suggested that the Democrats’ stand on reproductive rights would have led to the abortion of Jesus Christ. When a woman accused Duke University lacrosse players of rape, he derided her as a “ho,” and when a Georgetown University law student supported expanded contraceptive coverage, he dismissed her as a “slut.” When Barack Obama was elected president in 2008, Limbaugh said flatly: “I hope he fails.”
He was frequently accused of bigotry and blatant racism for such antics as playing the song “Barack the Magic Negro” on his show. The lyrics, set to the tune of “Puff, the Magic Dragon,” describe Obama as someone who “makes guilty whites feel good” and is “black, but not authentically.”"

Limbaugh was referring to how he thought Fox did not take his Parkinson's medications before appearances to make his symptoms worse. So he was not mocking the disease or Fox for having it.
Now you can get upset about him accusing Fox of being dishonest like that (deceptively not taking his medications) when he didn't know for sure if it was true. But it's a lie that he was mocking the Parkinson's.
So I will bigly, hugely admit that I was wrong, and I will apologize to Michael J. Fox, if I am wrong in characterizing his behavior on this commercial as an act.”
Then Limbaugh pivoted to a different critique: “Michael J. Fox is allowing his illness to be exploited and in the process is shilling for a Democratic politician.”

Look, let me put it to you this way: the NFL all too often looks like a game between the Bloods and the Crips without any weapons. There, I said it.
They’re 12 percent of the population. Who the hell cares?
[To an African American female caller]: “Take that bone out of your nose and call me back.
Have you ever noticed how all composite pictures of wanted criminals resemble Jesse Jackson?
Holocaust? Ninety million Indians? Only four million left? They all have casinos — what’s to complain about?
When a gay person turns his back on you, it is anything but an insult; it’s an invitation.”

Evidently you missed the actual context above that repaints those quotes in a very mundane, non malicious light.
Knee-jerk rush-posting is never a good idea, you tend to miss contextually important posts while rushing to hit submit, thinking you got a good zinger in. Except you didn't, because the bubble was already popped before you got here.
