Proof explosion happend inside reactor?

Take a look at this video released the other day. Pay close attention to the video at 2:59 and on.



It shows what I think is the reactor lid with its bolts blown out. You can see many bolts sticking out and some are bent. If what I'm seeing is correct then this reactor blew out from the inside and the lid did not hold completely. There even appears to be a whole in the base below the lid.

Anyone else seeing what i'm seeing?

At 2:57 you can see a peice of white crumpled debris under the lid on the left side, clearly the entire lid has been detached. To me it is clear the lid is not where it is supposed to be.

Edit, after reading the tube comments, someone said this is #4 and they had that lid off during routine operation and a crane inside would have been attached to it so that makes sense.

Ahh, missed that.

Does anyone know if the fuel removed from reactor 4 was "spent" or was it supposed to be re-inserted after the planned maintenance?
