Unfortunately, it's not that uncommon for bacterial contamination to happen, and for a major recall to be issued due to the contamination.
Last year, a bunch of acne gel got recalled due to contamination with the very same bacteria. (See link below.)

While I definitely think the Baxter incident is evidence of something serious and sinister, I think this incident is unfortunately just another
example of mistaken contamination.
Of course, I could very well be wrong, though.
