The transformer is the most efficient electrical machine known at up to around 95% efficient in the best cases. Switchmode power supplies can approach those figures as well but reliability and life expectancy is their downfall when compared to a simple mains transformer. Apart from 5% lost due to hysteresis, copper losses and magnetic fringing, the input power makes it to the output. Think of a transformer as an 'electrical gearbox' IE 220V, 50W in you get almost 50W out at whatever voltage the secondary is wound for and energy is not discarded.

50W in @ 220V~ (RMS) means the primary current is 227mA (RMS)
50W x 0.95 = 47.5W out and @ 12v~ output the secondary current will be about 4A
