The update was meant for sometime in April 2018 I checked my system settings and now, somehow, it says the device is up-to-date.
Here how I got FoNV to work:
Launch new vegas from the launcher
Alt + Tab out and press Ctrl + Shift + Esc to access the task manager
Go to 'Processes' tab and Find FalloutNV, right click it and click 'Go to details'
You should now be in the details tab with FalloutNV.exe highlighted. Right click it and choose 'Set affinity'
In the Processor affinity window, uncheck and check only 'CPU 0'
Click ok and tab back into the game.
You have to remember to reset the processor affinity to only 1 core every time you launch the game. There might be a way to force it to launch with only 1 core every time but I don't know how to do it.
Install NVSE[nvse.silverlock.org]
create folder name NVSE in Data folder
create nvse_config.ini open it and put in this line
[Memory]
DefaultHeapInitialAllocMB=400
then install NVAC[rd.nexusmods.com]
if still experiencing stuttering try NVSR[rd.nexusmods.com]
and edit fMaximumFPS = 30 or 60
and iHeapSize = 400
both lines are in the mod ini
and both mod are install inside NVSE folder
*These line must be edit in Fallout.ini in My Document only!

Download Mod Organizer 2 and get New Vegas Anti Crash and the 4gb patch. You'll also want/need the New Vegas script extender. I play NV moderately/heavily on W10 with no probs
Once you get the NVSE launch New Vegas using only that. MO2 should find it for you
