It's simple. The one telling the lie is pushing the lie. If the reporter or news outlet that was told the lie continues to publish that lie, even after it is proven to be a lie, they would be the ones pushing fake news.

But oh no,,, it can't be the source of the fake news... it has to be the fault of the news organization for not properly vetting their source.
