I think this is a hoax, but for what it's worth, one "Lisa Bardack" is mentioned in one of the email leaks:

Hillary, in an interview back in 2014, said she would "absolutely do what other candidates do" and release her medical records:

politicalticker.blogs.cnn.com...

Obviously, she has not released her records yet. She has something to hide.

ETA: I don't think those records are real. We can only hope we do get the genuine ones.

Clinton did release her medical records and this one is real

I can't believe what that says.
"She eats a diet rich in lean protein, vegetables and fruits. She exercises regularly, including yoga, swimming, walking and weight training."
^^She can't walk up a flight of 6 stairs without assistance and we are supposed to believe that crap? ^^

Note her weight listed in the report......

That report was from last year. The campaign trail takes its toll.

When the press was demanding McCain release his medical records during his presidential campaign, McCain let reporters view his medical documents. There were 1,500 pages released of both medical and psychiatric records:

www.nytimes.com...

McCain released his records amid concerns he had the temperament to be president.

Has Hillary let the press see hers?

I agree. I have epilepsy and I am treated by a neurologist. I also work in medicine....and I can guarantee that I would NEVER willingly violate HIPPA laws unless I were NEVER EVER going to work in medicine again and I were 100% sure my name would NEVER be found out....and if it is NOT a HUGE University based clinic...well...you ARE going to be found out....I work with Epic and it is almost IMPOSSIBLE to get information like this print out without using a password and log in credentials. As well, print outs from a lot of EMR systems do not look like this. I see this kind of "report" from practices that refuse to use EMR. AND, as a multimillionaire and former first lady, Secretary of State etc etc...she would get the BEST of the BEST care that money could buy.

I dunno..... I DO think she has some kind of neurological issues; I have them from multiple concussions and possible brain damage from untreated grand mal seizures, (ok, so I was stubbornly dumb about taking medications and I used to have a grand mal seizure every month for two years before I got smart and started to take the medications). I have similar head movements when I have a "small seizure"...the petit mal ones.....I have trouble swallowing at times, (I choke on water, food, even my own spit when things get bad)...I stutter, have outbursts of anger and crying jags......and I can barely work....in fact, I have essentially retired at age 50...from seeing patients as patients were starting to notice that something was wrong with me....I walk funny sometimes, can easily lose my train of thought and my memory is almost gone...especially short term. I jump at loud noises and if I am in a room with loud noise, lots of people and too much stimulus....I can have small seizures.

at any rate...from a PERSONAL perspective from PERSONAL experience with neurological issues...I am 100% convinced she has a problem....however, I 98% feel this "medical record" is a hoax....

my two cents, remember......

Yeah, it must be really tough. She can't walk up stairs without two people to hold her up.

The DOB is one day off. I would think her doctor would get that right.

You would be downright SCARED if you saw the mistakes I have caught in patient's charts....I have even seen doctors put down information about the RIGHT eye, in the LEFT eye column, give a patient a +2.00+100 x 180 rx when it should have been a -2.00 +1.00 x90 (a HUGE difference, by the way). I have seen LOTS of mistakes, especially using drop down menus on EMR programs.......PLUS, it isn't the doctor who normally puts in that information (DOB, etc) it is the front desk person or the technician/assistant/nurse.....

Exactly. This information was faked, and its purpose may have been to try to make Trump supporters look bad.

Or - the documents could have been created by someone on these forums, since it does repeat some of the information in recent threads. In that case, they may have just wanted attention and some laughs.

There is no way that fakes were ever going to be used to actually stand against Hillary, because they could be proven as fakes.

It is a shame.
There is no need for fakes and hoaxes when Hillary herself is all the proof that a person needs that she is not healthy.

I'm not going to click the link, but if it is the same document linked by reldra above in the news article, then it proves that the documents in this OP are faked.
For the reasons I mentioned previously regarding the Dr.'s middle initial, title, etc.

I work for a university, but our fax number is just one digit off the fax number of a regional healthcare organization's home health office. I can't tell you how many faxes I've received containing patient information and medical reports. I don't know whether the letter/report in the OP is real or fake, but I can tell you that personnel in the medical profession are no more accurate/precise in their grammar and punctuation than any other industry. I see misspellings and bad punctuation all the time. I see patient information (that is HIPAA protected) and full names all the time. Most medical offices do their best at protecting patient privacy, but if someone wants to leak something it gets leaked...or in my case, accidentally broadcast. Nothing in what I read here screams fake...but that doesn't mean it isn't.

I'm not so sure those issues you've highlighted 100% point to this being fake.

A cursory Google search returns the following result:
Lisa Bardack employee profile

On this employee listing for her, they DO use her middle initial. If you scroll down, you will see the following under the Biography section:

- Chairman of the Department of Medicine, CareMount Medical

This matches the title in the alleged leak documents exactly. Except wait, CareMount Medical is not the same establishment listed in the supposed leaks, which is Mount Kisco Medical Group. Or is it?

Another cursory Google search for CareMount Mount Kisco Medical returns the following:

Mount Kisco Medical Group to become CareMount in early 2016

Looks like these items on the leak may not be as suspicious as originally thought. The DOB is a different issue.

Where are you seeing this? I see the same date on the document that I find elsewhere, 10/26/47, even on wikipedia.

It's fake, for one important reason, it should have time stamps on all the sections and tests as well as the prescription indications.

The twitter account did have a tweet stating that he or she was a laid off worker from Mount Kisco.

Looking around I found this:
Mount Kisco Medical Group laying off 109 workers

There could be some bit of credibility to this.

Similar examples were posted throughout the thread, check them out. This type of document seems to be some sort of doctor's note to give out to "whom it may concern", no detailed prescription information would be there.

I found some medical records on line that look similar

Could you post any? So we can compare.

Look up on line examples of medical records for beginning transcriptionists. They are generally far more detailed, and would be especially so for a high profile patient. This one is so short, it could be for a kid with tonsillitis. That's not to say I don't think both candidates don't have both mental and physical issues sufficient to derail them; not to mention moral ones.

And why is no one discussing Trump's ludicrous physical exam letter? Don't go to the doctor's dr's www address, it's a virus.
