Computers do most of the work for them.
Once the computer establishes a consistant use of key words, then an operator will monitor a few of those persons phone calls.
I seriously doubt my parents ever discuss anything that relates to the stuff we discuss on ATS, so they would not have an operator monitor any of them.
They way you can determine if someone is monitoring your phone call is you can hear your voice echo. (But this doesn't happen all the time).

as far as I was aware, the systems used to monitor infomation in the us, was based in the uk (GCHQ - commonly called the doughnut) Its primarily used for Sigint, but i think the US use it to monitor. I could however, be completely barking up the wrong tree!!
