I believe what we are seeing is a chosen doppelganger for Diana to morph into.
The original Melania was very very boyish.
Very flat chested unlike Diana and the present Melania.
Of course there are boob jobs. But maybe not needed in Diana being quite buxom and we can just assume it was a boobjob.

That’s what I’m assuming, but you never know and I try not to jump to conclusions.
