Nuclear Terror

If terrorists got their hands on a nuclear/biological weapon what are the chances of it being smuggled into the country, transported and detonated in a populated area.

Also if this did happen, what would the US governments responce be, its not like they can send a nuke back to Afghanistan, like they could if N. Korea launched one.
