I think it has to do with state of mind, heart and soul at the time of death, karmic debt, etc, but even Jesus said that if you end up jailed, you won't get out until you've paid the very last penny..

That said, some people develop totally corrupted minds and hearts which continually keep on doing the same loop over and over again ad infinitum. Hard to see how they would escape that unless a higher power were to step in there and help them to see, but even then, there are I'm sure those who refuse to see, and who would reject in a countless series a God of love, so it's possible perhaps that such a person would never escape their torment.
