Opened briefly to add a few things...
Usually, a member is warned a few times and/or post-banned (temporary ban) for repeated or gross violations of the Terms and Conditions. Note, these
violations could ALSO be in chat, U2U's, etc., so may not be viewable to the membership at large.
Perma-bans (permanent bans) usually result from not cleaning up one's act after a post-ban, or even one-time gross violations of the T&C if severe
enough. (for example, threatening to kill someone or overly crass sexual comments to a member, etc.). While a post-ban is typically done by one mod,
perma-bans are generally discussed prior to doing so, unless blatantly obvious (but sometimes even then).
Just be assured, if someone is banned, there was a good reason for it, and it wasn't for anything trivial.
