I just finished the report. The basic mistakes made were:
- they were wearing too much wieght (60 lbs each)
- their "tenders" were NOT qualified to control the decent lines
- the "tenders" misunderstood the control signals
- the "tenders" had been drinking beer!
So they dropped FAST to around 200 ft when their initial plans (again, not read by their tenders!) were to only hang out at 20 ft!!
The gross part is that they were also pulled up WAY to fast. Their lungs burst on ascent and significant bleeding occured.
