Supposedly happened during test in Nevada

Steel cover over shaft for underground test was launched into space, supposed to have reached escape velocity

There are two schools of thought on it. One is that it reached escape velocity and is somewhere out in space, the other is that while it was launched into space, the heat and pressure involved caused it to melt somewhere on the way up and it never got out of the atmosphere.
