12 feared dead in military helicopter collision off Hawaii coast - report

The Commanding Officer of HMM-463 was fired three days before the crash occurred. He had been in command for less than a year, and had failed to maintain acceptable standards of readiness for the unit. There was one instructor pilot paired with one student pilot, and two crew chief instructors paired with two students on each aircraft. It was reported that the unit was far below normal on flight hours in recent months.
