eta: To clarify, I don't think a conflict between the gender you experience within, and the biological gender is delusional, because it happens. My comment is directed towards the 'six-year-old girl" aspect of this. He doesn't seem actually delusional to me. This seems more like attention seeking, and I would personally discourage it's encouragement.
