Oops that is a classic screw up. Characters panicked most likely, almost unforgivable pilot error

Not necessarily. British Midlands was unfamiliarity with type. Or, as I said they might have had a problm with #1 on the flight in so they reacted to the emergency expecting the other engine. Panic doesn't necessarily have anything to do with it.

TransAsia is being forced to redeploy A320s on ATR routes, because of bad crew evaluations. So far 49 pilots have gone through the evaluation process, with 10 requiring "strengthening" of their capabilities. Another 19 pilots have to go through the process.

The probe indicated that the crew should have aborted the takeoff. About four seconds into the takeoff roll, the pilot in the right seat called out that the automatic takeoff power control system (ATPCS) was not armed. The other pilot acknowledged the call, and continued takeoff. Just after the 70 knot call, he noticed the system was armed. At 34 seconds after takeoff, the system automatically commanded a power increase to the #1 engine, and began autofeathering the #2 prop, which is the normal response to a #2 engine failure. The crew then proceeded to shut down #1, leading to the crash.
