On 1st December, 2011, the official switch-over occurred after the Operating Certificates were merged, as part of the United-Continental business merger/stock swap business transaction.

From this source, the recordings of the last flight to use the "Continental" radio call sign with Air Traffic Control (at Newark Airport, in this instance) was Flight 86 "heavy", a Boeing 777-224 from Shanghai, China. (ZSPD)

(The "heavy" in the call sign is added for all jets capable of weights of 250,000 pounds or more).
