I think that's a bit naive really, who the hell would choose to do that job?
A lot of people are not in a position to choose their job, or career, they have to take what they can, for many different reasons. The system we live in is set up purposely to make choices limited, it makes us easier to control without overt force. For example, if there was an abundance of 'jobs', and we could all really choose our professions, we would have power the employers. The employers would have to constantly raise wages, and improve working conditions, because the workers could just go get another job if they were dissatisfied. The 'owners of the means of production' are not going to let that happen, thus we get unemployment. An artificial scarcity of 'jobs' in order to maintain control over labour.
Those that truly get to choose their job are the lucky few, the media helps to makes it appear that we all choose our positions in life.
And as far as having the 'freedom' to choose to move if you don't like the social structure where you are is pure BS.
How many have the financial ability to quit their job and move to another place? Moving to a new place, especially a new country, is VERY difficult, even more so if you don't have much money. I know, I did it twenty five years ago. It's not just financially difficult, but psychologically also.

However, in your first point, you do realize that EMT's attend quite a bit of schooling, right? The seatbelt laws...they are not that old. Maybe early 90's? Before that, there were still people scraping peoples brains off the pavement. Some folks just aren't bothered by that sort of thing.
The reason you and I are required to wear seatbelts is because the insurance lobby was able to convince the Politico's at that time that we needed to wear seatbelts. It is all about reducing risk for the insurance companies. That is all.
If "they" gave a damn about your health, we would see things like viable cancer treatments being approved, and a cessation of the medical testing that is done on US citizens via mass sprayings.
